Also, a valid and current copy of the driver’s Insurance Declaration must be on file in Stanley Office.



HOW TO CREATE YOUR “BE A MENTOR” ACCOUNT

To complete the clearance process via the Be A Mentor portal, you will need:

  1. State-Issued Driver’s License or State Identification
  2. Tuberculosis (TB) Clearance:
    1. Proof of TB clearance within the last four years is required.
    2. If you need TB clearance, consult your medical provider or find community testing options. Or access the website https://www.cchealth.org/health-insurance/my-contra-costa-health-plan/provider-directory
    3. More information on TB testing is available from Contra Costa Health Services.
    4. If you do not have access to previous TB screening results submitted to your school, you may pick up your proof of clearance at the District Office on certain specified dates this summer. Details will be provided by late June.
  3. Live Scan Fingerprinting:
    1. Use a provider of your choice. Local providers can be found here or on the website: https://oag.ca.gov/fingerprints/locations
    2. Download this Live Scan form and take it to your local provider.
    3. If you completed Live Scan for LAFSD in the 2023-24 school year, repeating this step is optional if you create your Be A Mentor profile before August 31, 2024.

How do I know that I am cleared to volunteer?

When you log into your Be a Mentor account, under My Requirements, you can check whether all requirements have changed from incomplete/pending (yellow) to completed (green) which would indicate that you are done and have been cleared. 

 

You should also receive an email with the subject LAFSD Volunteer Clearance that indicates you have completed the requirements and are cleared. 

 

If you’re unsure, you can also open the Dashboard in your Mentor account and look in the bottom right corner under Projects/Activities, Most Recent Changes and you will see Volunteer Chaperone/Driver: Status is changed to “Cleared”.
